Must live within 30 miles of Canfield, Ohio
Limited to one necklace per person and address
Must have personally experienced pregnancy or infant loss, or intend to gift it to someone who has

Necklaces will be mailed within 5–7 days of submitting the form, directly to the mailing address provided.
