 In order to promise the highest quality, individualized and necessary approach to your treatment, I have chosen to be an out-of-network provider with private insurance companies. Contrary to popular belief, this out-of-network therapy or private pay approach can be more cost-effective than your typical in-network insurance therapy approach as you will require less frequent and fewer visits to reach your goals.

Pricing

90 min. Initial Evaluation - $175

60 min. Treatment Sessions – $125

Clients are typically seen an average of 5 - 8 treatment sessions total over the course of 2 - 6 months, in order to experience good outcomes.

Therapy sessions are always one-on-one with me for the full hour, and take place in a relaxing treatment space. Babies, children or family members are welcome to be present, if you so desire.

Payment Options

Flexible Spending Accounts (FSA) and Health Spending Accounts (HSA) and Health Shares are all accepted and very welcome forms of payment.

Credit cards, debit cards, cash, or check are also accepted forms of payment.

Insurance and Reimbursement Options

Itemized receipts, or “superbills”, are available for patients to submit to their private insurance for out-of-network reimbursement. Usually, patients are only eligible when a certain out-of-network deductible is met.

Not a Medicare or Medicaid provider. Due to federal law, Medicare and Medicare Replacement Plan cannot receive “skilled therapy” with Calko Pelvic Rehabilitation, even if you offer to pay cash. There are other pelvic floor therapists who accept Medicare in the region to whom I can refer you.
